When an incident occurs, a survey response email will be sent to the registered email address.
Click "Report Impact Status" to begin the survey.

A list of sites that may have been affected by the incident will be displayed. Check the sites you wish to report on and click the "Submit" button.
You can select multiple locations. To respond for all displayed locations, check the box at the top (next to "Locations" in the image) to select all displayed locations at once.

After confirming that the selected sites are correct, please select whether the site was affected based on the situation.

After selecting the risk impact, you can leave a comment. Please add detailed information about the site’s status.
You can also use templates for your responses. Please use them as needed.
